Hall
of Fame
In 2002, the Ottumwa Regional Health Center’s 50th
Anniversary Committee determined that those persons whose dedication
and
achievements
have significantly affected the Health Center, Regional Ventures,
Inc., and health care in the regional community should be honored
for their contributions. The Foundation’s Hall of Fame, located
on the first floor of the Health Center, was established to provide
a permanent place of honor for these persons.
Mission
To honor in perpetuity those individuals, families or groups
whose contributions forever changed the landscape of health care
in Ottumwa and the regional community.
Inductions
Inductions shall be limited to no more than three (3)
individuals and shall occur on an annual basis.
Criteria Guidelines
• Nominee may be living or deceased
• Nominee has demonstrated support for ORHC through sustained contributions
as an employee, volunteer, physician, board member or benefactor
• Nominee has selflessly given time, effort and talent for the benefit
of health care in the regional community
•
Nominee leaves a legacy of achievements that have (or will) profoundly
influenced health care in the regional community and have (or will
have) enduring value beyond the nominee’s lifetime
Eligibility
Individuals, families or groups associated with and/or
having a direct relationship with Ottumwa Regional Health Center
are eligible for nomination. Achievements and/or qualities honored
are to the degree that the nominee’s efforts advanced health
care in the region and on the indelibility of that impact.
Nominations
• Nominations will be accepted between the first week of January through
the second Friday of February
•
Nomination forms will be available from the Foundation office and
the Foundation’s website
• A nominee must have been involved in one or more of the following:
The creation of a vision and its ensured development
The development of financial growth and solvency of the ORHC
The personal engagement and persuasion of others to engage in a noteworthy cause
The development of an enduring milestone or significant turning point in history
or in the future
of health care in the region
Selection
Foundation Staff will review nominations for accuracy and completeness
and present them to the Selection Committee for consideration. The Committee
will be comprised of members of the Foundation Board, Health Center personnel
and other individuals having a longstanding history with the Health Center. The
Committee will discuss the nominations and each member of the Committee will
have the opportunity to vote on the nominee(s) they feel are most worthy of induction
at that time. The nominees receiving the most votes will be the Hall of Fame
honorees for that year. In the event of a tie, the President & CEO of the
Foundation will cast the deciding vote. Nominees not selected will be brought
before the Committee the following year. The selection process will occur in
February of each year.
Presentation of Inductees
Hall of Fame inductees will be formally recognized
and honored as part of the Foundation’s annual Chairman’s Reception
in May of each year. Inductee will be memorialized with a plaque on the wall
of the first floor hallway at the Health Center.
